Enrolment Process

We are currently accepting new patients.

Enrolling at Queen Street Doctors means you choose us to be your primary provider of medical cre. The advantages of enrolling at a general practice are cheaper consultations with the doctor, gaining access to a range of other services and receiving continuity of care.

To enrol, you are required to complete our Enrolment Form, Intro Health Questionnaire and provide evidence to ensure you are eligible to enrol.

You are eligible to enrol if you are a New Zealand Citizen, Permanent Resident or on a consecutive Work Visa that is valid for at least two years. You also have to be living in New Zealand to be entitled to receive the benefits of an enrolled patient.

New enrolments are processed by the Ministry of Health the month after you have enrolled. This means there will be a delay in receiving government funding. Once you are fully enrolled and funded, the practice will start applying the enrolled consultation fees.

Fees

The cost of your first appointment will be charged at the casual rate of $110.00. All subsequent appointments will be charged at the enrolled rate:

Enrolled Rates for standard 15min doctor appointments are:

Age 0-14     Fee: $0.00  
Age 14-17   Fee: $40.00 
Age 18-24   Fee: $60.00 
Age 25-65   Fee: $70.00  
Age 66+      Fee: $65.00    
CSC     Fee: $19.50

Extended Consultations are $45.00.

Non-Resident Fee: All appointments $130.00 (not eligible to enrol).
